残疾人服务

完成 Request for 残疾人服务 for Class Accommodations form. To submit your Guardian request you are required to attach the following:

• Documentation of disability (required for NEW requests, RETURNING requests can submit a copy of class schedule)
• Copy of your photo ID (i.e. State ID, Driver’s License, or School ID)

Here is a sample list of eligible documentation:

• High School Special Education Records (504, IEP) no more than 5-years-old, 根据地区和全国范围的文件指南，诊断为ADD/ADHD的IEP或504住宿不能超过3年.
• Medical Record with Disability Diagnosis
• Mental Health Diagnosis
• Any Military/Veteran Records Stating Disability

阿拉莫大学 Form for Interpreting Services

For online, dual credit classes taught by St. 菲利普的大学 faculty, please follow the “How to Request 残疾人服务 for Class Accommodations”的过程.

For classes taken at St. 菲利普的大学, students may request services by following the “How to Request 残疾人服务 for Class Accommodations”的过程.

A student must request Classroom Accommodations each semester enrolled by completing the Request for 残疾人服务 for Class Accommodations form.

Accommodations are not retroactive.  Accommodations begin the day the letter is emailed to your St. 菲利普的大学 professor(s).  学生有责任主动安排他/她与每位老师的住宿.  当老师收到你的住宿信时，对话就开始了，并且应该持续整个学期.

Under both the Americans with Disabilities Act, 最近修订的, and 第504节 of the 1973 Rehabilitation Act, reasonable accommodations are provided to qualified students with a disability.  如果住宿从根本上改变了课程或项目的交付方式，则住宿不合理, results in an undue burden, or possesses as a direct threat.

In the college environment, we do not contact parents, 修改任何作品, or alter any program or curricula.  Students are expected to advocate for themselves, seek feedback from faculty, must give permission for anyone to obtain information about services provided or grades, and are expected to meet the expectations of each course.
